Fill in the account form and either bring it into Zap or send it by fax at 721.8728.

UVic accounts please fill in the top and right hand side of the form. Include names of anyone that will use the account because we cannot let anyone use the account if they are not listed on the Account Form.

UVSS Clubs and Course Unions please fill in the top and left hand side of the form. Once the form is filled in have it signed by the appropriate Board member responsible for Clubs or Course Unions.

Processing time for accounts is 24 to 48 hours.
